Enrollment Procedure

STEP 1: Admission Office

Secure an admission slip and registration form then submits the following enrollment requirements:

Old Students

  • Admission Slip
  • Registration Form
  • Original copy of Report Card

Transferees

  • Admission Slip
  • Registration Form
  • Original copy of Report Card
  • Good Moral Certificate
  • Certificate of Transfer

New Students

  • Admission Slip
  • Registration Form
  • Original copy of Report Card
  • Good Moral Certificate

ESC Grantees

  • Admission Slip
  • Registration Form
  • Original copy of Report Card
  • Good Moral Certificate
  • Grantee Enrolment Contract

SHS Voucher Recipients

  • Admission Slip
  • Registration Form
  • LRN – Learning Reference Number (for Public only)
  • QVR Certificate (for Private only)
  • Original copy of Report Card
  • Good Moral Certificate

STEP 2: Registrar’s Office

Submit the accomplished registration form for encoding.

STEP 3: Cashier’s Office

  1. Get an Assessment of payment
  2. Pay tuition and other fees (Cash or check)
  3. Pay PCA fee and school org/club fees).
  4. Get Official Receipt

STEP 4: Finance Office

  1. Order the following:
    • School uniform cloth
    • Group Color Uniform
    • SHS polo shirt
    • Books and Notebooks
    • RFID
  2. Proceed to Cashier’s office again and pay your order.

STEP 5: Supply Office

  1. Present your payment receipt
  2. Pick-up your materials at the Supply office according to schedule.
